WHO South-East Asia Region polio-free, accelerated efforts needed in view of increased risks: experts

The WHO South-East Asia Region sustained its polio-free status despite disruptions caused by COVID-19 pandemic but needs to scale-up measures in view of the persisting and increased risks of polio globally.

“Countries have been making efforts, but gaps persist, specially at sub-national level. We need to scale-up surveillance and immunization coverage and update our polio outbreak response capacities to respond rapidly and timely in the event of a polio outbreak,” said Dr Poonam Khetrapal Singh, Regional Director WHO South-East Asia as the South-East Asia Regional Certification Commission for Polio Eradication met here to review the situation in the Region.

The chairs of national certification committees and global certification commission; representatives of donor and partner agencies and WHO participated in the two-day meeting earlier this week.

The Commission concluded that the Region has not had any case of wild polio virus or circulating vaccine-derived poliovirus (cVDPV). However, risks persist due to continued circulation of wild poliovirus type 1 in endemic countries bordering the Region, as well as due to the cVDPVs being reported from several countries of other Regions.

“The situation requires constant monitoring until global polio eradication is achieved,” the Commission recommended.

The Regional Director said globally polio remains a Public Health Emergency of International Concern. Earlier this month, New York declared a state of emergency following the detection of poliovirus in wastewater samples. In recent months cases of wild poliovirus type 1 have been reported in Afghanistan, Pakistan, Malawi and Mozambique.

In June, the Thirty-second Polio IHR Emergency Committee assessed the risk of international spread of cVDPV2 or circulating vaccine derived poliovirus type 2, as high, in view of COVID-19 outbreak impacting routine immunizations services in many countries. cVDPVs are a risk in areas with low immunization coverage.

“Today, coverage in the Region of the bivalent oral polio vaccine and inactivated polio vaccine has for the most part stabilized or improved since 2020; however, the trajectory in several countries continues to be cause for concern,” Dr Khetrapal Singh said.

Though the Region was able to maintain global surveillance standards throughout the COVID-19 response, there are variations at national and sub-national levels that need to be addressed, she said.

Emphasizing on sustaining adequate resources amid immense fiscal stress, Dr Khetrapal Singh said, enabling polio networks in the Region to strengthen immunization systems and carry out other public health functions will help polio-priority countries maintain core capacities and infrastructure until global polio eradication is achieved.

“The last year two and a half years – almost three – have not been easy, but the Region’s continued polio-free status is a testament to the dedication and commitment of polio programme workforce, many of whom have significantly contributed to the COVID-19 response, and the steadfast resolve of donors and partners,” the Regional Director said.

At the two-day meeting on 21 and 22 September, updates on polio situation were also shared from WHO Africa Region, WHO Eastern Mediterranean Region, WHO European Region and WHO Western Pacific Region.

USAID announces countries for test-to-treat and oxygen programming as part of the United States COVID-19 response

Today, at the Global Action Plan (GAP) Ministerial on the margins of the 77th United Nations General Assembly, the U.S. Ambassador to the United Nations Linda Thomas-Greenfield announced that USAID has advanced its programming in two important areas across twenty lower-and middle-income countries. The first supports the introduction of “test-to-treat programming” for populations at high-risk of severe illness from COVID-19. The ten countries announced today include: Bangladesh, Botswana, Côte d’Ivoire, El Salvador, Ghana, Lesotho, Malawi, Mozambique, Rwanda, and Senegal.

The second initiative will expand access to lifesaving oxygen supplies in more than a dozen countries. These have been identified as Côte d’Ivoire, Democratic Republic of Congo, Eswatini, Ghana, Jamaica, Lesotho, Malawi, Mozambique, Namibia, Nigeria, Papua New Guinea, South Africa, Tanzania, Vietnam, and Zambia.

Test-to-Treat Programming Launches in Ten Countries

Test-to-treat programming prepares health systems, health workers, and communities to make rapid COVID-19 testing broadly accessible for people to get tested as soon as they develop COVID-19 symptoms. It then ensures that those at highest risk of severe illness can access key oral treatment options early in their COVID-19 illness to prevent severe disease and hospitalization. Integrating test-to-treat programming strengthens health systems’ capacities to quickly identify new infections and initiate appropriate treatment in high-risk patients. Treating sick patients while they are still at home and before they get too sick can reduce severe COVID-19 illness and death, and reduce the burden on health systems. At the Second Global COVID-19 Summit in May 2022, USAID had announced a $20 million investment in piloting test-to-treat programming and other ancillary support.

USAID’s announcement that it and its implementing partners will be working with Ministries of Health, civil society, healthcare workers, and others such as The Global Fund in the ten identified countries to pilot test-to-treat programming marks exciting progress. The best practices and lessons learned from these efforts will help countries develop plans to scale up their programs on a national level, and can be shared with countries around the world, including the United States. The pilots will position countries to act quickly in the case of a surge in COVID-19 cases.

Expanding access to medical oxygen

As a global leader in supporting increased access to lifesaving oxygen in health facilities that lack this critical medical supply, USAID’s holistic approach to expanding access to oxygen focuses on health systems needs. This includes training healthcare workers and building the engineering and logistics capacity of countries to operate and maintain these oxygen systems for the long term.

USAID is building off the $50 million investment to expand global access to medical oxygen and announced that this work will support facilities across Côte d’Ivoire, Democratic Republic of Congo, Eswatini, Ghana, Jamaica, Lesotho, Malawi, Mozambique, Nigeria, Papua New Guinea, Tanzania, Vietnam, and Zambia to build and improve healthcare systems that support the use of bulk liquid oxygen.

Through previously-announced COVID-19 funding, USAID is also employing market-based approaches that lower the price of oxygen. By working with the private sector, these approaches aim to make the future purchase of oxygen more affordable for governments. This market-shaping work will start in Democratic Republic of Congo, Eswatini, Lesotho, Malawi, Mozambique, Namibia, South Africa, Tanzania, and Zambia.

USAID’s work to increase access to oxygen will save COVID-19 patients, and it will have lasting benefits beyond the COVID-19 pandemic. The United States’ commitment to increasing access to oxygen will also support other global health programs, such as those that address child and adult pneumonia, safe birth, safe surgery, and new infectious disease outbreaks.

Stiffer penalties for human traffickers in Tanzania

DAR ES SALAAM— Tanzania has approved amendments to the local anti-human trafficking laws to include tougher penalties such as lifetime jail terms and fines of up to Tsh200 million ($86,000).

Attorney General Dr. Eliezer Feleshi said the country needed to implement punitive measures to effectively shut down the vice.

The Human Trafficking (Amendment) Bill fixes a minimum jail term at 30 years for offenders of trafficking and hands judges free rein to impose a life sentence depending on the nature of the case.

“We have amended section 5(4) which showed that a convict may be jailed for 10 to 20 years to a minimum imprisonment of 30 years depending on the nature of the case,” the AG said.

Serial human traffickers may be handed life sentences while first offenders would pay a Tsh100 million ($43,000) fine.

Najma Murtaza, the deputy chairperson for the Standing Committee on Constitutional and Legal Affairs had suggested a tougher punishment to repeat trafficking offenders (serial traffickers).

Another MP, Salome Makamba said human traffickers were wealthy people, hence the need to hit them hard with large sums in fines.

“I want us to slap bigger fines because these are people with no economic problems,” she said.

Human traffickers are paid between $5,000 and $15,000 to transport a single person, mostly from Ethiopia and Somalia, through Kenya, Tanzania, and Mozambique to southern African states, a bulletin by the International Organisation for Migration (IOM) indicates, suggesting the existence of networks in these countries who collude to traffic people.

The IOM estimates that over 15,000 illegal immigrants pass through Tanzania every year, mostly from Ethiopia, Somalia, Burundi, and Rwanda on transit to South Africa and its neighboring states.

Other key destinations for their human cargoes are Oman, Saudi Arabia, United Arab Emirates, India, and China, data from IMO showed.

Victims of serial traffickers are women and children for domestic work (household), crop farms, mines, and the informal business sector.

Tanzania is a leading transit route for trafficking due to its geographical position and longer and porous land borders and the Indian Ocean routes with pirate ports in Zanzibar, Tanga, and Mtwara.
